ACCIDENT INVESTIGATION AND CAUSE ANALYSIS

Scope of Work: Investigation and analysis of fatalities, injuries, and property damage accidents. Identification of actions and conditions contributing to the accident, analysis of root causes, and development of effective preventative measures.

BEHAVIORAL SAFETY: Identification and development of the critical employee safe work habits required for achieving and sustaining accident free operations.

SERIOUS INCIDENT PREVENTION: Identification of the critical actions and processes required to prevent serious, high consequence accidents including those involving fatalities, major property damage, fires, explosions, hazardous material releases, transportation incidents, and other incidents.

Our investigations include the use of sophisticated computer software such as The University of Michigan's 3d Static Posture Assessment Program that analyzes the effects of lifting and other manual tasks on the back. These assessments which use 3-dimensional modeling provide a scientifically-based quantified comparison of the forces involved in performing specific tasks to human physical capabilities and limitations.

HAZARDOUS MATERIAL SAFETY

Handling and storage (tank storage, warehousing, underground salt dome storage), loading and shipping (rail tank cars, tank trucks, drums), pipeline operations, Process Safety Management (PSM), EPA Risk Management Plans (RMP), Emergency Response Plans.

PREMISES SAFETY

Evaluating and analyzing the risks related to the design and maintenance of facility features such as stairs, ramps, and walkways.

REGULATORY COMPLIANCE
